1.
Contacts supervisor in the event of an elopement, injury, or threatening condition.
2.
Participates as a team member to provide a nurturing and consistent therapeutic treatment environment.
3.
Develops a rapport with individuals and cultivate a safe and supportive relationship.
4.
Maintains client confidentiality at all times.
5.
Remains awake and alert at all times to provide supervision of clients and assistance when needed in activities of daily living including clients' personal hygiene, household chores, laundry, medication compliance and meal preparation.
6.
Participates in program activities and outings as assigned by supervisor.
7.
Provides program orientation to clients as assigned in a timely manner.
8.
Transports clients as assigned in a safe and timely manner.
Signs vehicle in and out per agency policy.
Completes inspection and documentation prior to driving.
Ensures that vehicle remains clean and in good repair.
IDD Supervised/Supported Living specific:

Assist with other areas of IDD service provision as assigned.
Documentation and Quality Improvement:

Documents relevant events in a factual, accurate, complete, and timely manner.
Follows correct case note format.

Documents client concerns in a clear and concise manner and presents to supervisor in a timely manner.
Interpersonal and Leadership Skills:
12.
Attends and participates appropriately in clinical staffings when requested to do so by supervisor.
13.
Responds to clients needs in an appropriate and timely manner.
14.
Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.
15.
Obtains shift report from departing coworkers.
Provides shift report to oncoming coworkers that is complete and alerts oncoming shift of relevant concerns.
Support Duties:

Completes additional job duties assigned by supervisor.
17.
Notifies an appropriate designated supervisor a minimum of two hours in advance of any program absence so that the supervisor can arrange program coverage if necessary.
18.
Monitors clients' belongings for items of safety concern and/or contraband.
Removes any unsafe or inappropriate items and reports removal to the program supervisor.
19.
Identifies and reports to supervisor in a timely manner any safety concern or any unusual incidents in the work environment.
20.
Screens visitors and telephone calls to ensure only approved persons are contacting residents as outlined on the client's visitor log and in accordance program policy.
21.
Completes housekeeping duties assigned by supervisor in a timely manner.

Compassionate about the care and well-being of othersRequires satisfactory completion of CPR and CPI within the first 30 days of employment.
Requires ability to be covered under company insurance to drive a company vehicle for client transportation.
Requires valid driver's license and current auto liability insurance.
Requires completion of annual training in compliance with DMH standards.
Previous experience in a mental health setting strongly preferred.
Previous experience working with SMI/IDD population in a treatment setting strongly preferred.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
